Digitalis electrocardiographic toxicity

M.G. 77yr male; post-ischemic cardiopathy discharged in digitalis treatment.




08.17.2001: ecgraphic tracing obtained at hospitalization discharge. The Patient had an ICD implantation for previous ventricular tachycardia some months before. It's evident a bradykinetic sinus rhythm, a minimal atrio-ventricular prolongation time, signs of previous infero-postero-lateral myocardial infarction. The repolarization alterations are secondary at the old ischemic damage.

 



 

09.19.2001 Ambulatory 24 days post-discharging ecg tracing. The Patient started epigastric urent disconfort, nausea and vomiting in the last 3-4 days. Digoxinemia were measured at 2,8 ng/ml. The ecg shows, beyond the atrio-ventricular prologation time and the signs of the old myocardial necrosis, an alterated repolarization aspect in V2-V3-V4 with a marked down concave ST segment and a biphasic T wave, consistent with possibile digitalis toxicity signs.
